Registration Forms

Please choose the correct residency form based on the information below. 

Residency Form A: Please complete this form if you are the legal parent/guardian of the student(s) you are registering and you have proof that you own or rent a home in the district.

Residency Form B: Please compelte this form and have it notarized if you are the legal parent/guardian of the student(s) you are registering and you live with a resident (friend/relative) within the boundaries of the district.

Residency Form C: Please complete this form (both sides) if you are NOT the legal parent/guardian of the student(s) you are registering and you have proof that you own or rent a home in the district. You MUST also supply a notarized letter from the legal parent/guardian of each student stating that you are authorized to make educational and medical decisions on behalf of each child.
